CHRONIC DISEASE
MANAGEMENT (CDM)

CHRONIC DISEASES afflict a greater number of Australians than ever before. These conditions are often lifelong, but with close and correct management by your GP, these conditions can be treated and controlled.
A Chronic Disease Management plan, or Care Plan, comprises visits to our practice nurse and doctor, and will often allow you up to five (5) Allied Health visits per year to assist in managing your chronic condition. These can include podiatry, physiotherapist or diabetes educator, to name a few. Ask your GP if you are eligible for a CDM plan at your next visit. These consults are fully BULK BILLED.

Separate to a CDM or Care Plan, Health Assessments are run at Kelso by our practice nurses, open to eligible patients only, and are fully BULK BILLED. These consults help to ascertain the ongoing health of our patient population and are generally undertaken once a year. Please ask your GP if you have any questions, concerns or to glean your eligibility for these assessments.

HEALTH ASSESSMENTS

DRIVING LICENCE ASSESSMENTS

Medical assessments for driving licence assessments are conducted here at the Kelso Medical Group. For pensioners, they are bulk billed, but if you require this for commercial purposes, please call the surgery to find out your fees. In most cases, commercial licence checks are not refundable from Medicare, but can be often claimed on tax or a benefit can be sought from your employer.

PHARMACIST

Our onsite non dispensing Pharmacist, De Benino is available to assist you with management of your medicines. A home medicine review is a BULK BILLED health service involving our pharmacist visiting you at home and reviewing all your medicines. The pharmacist then works together with your doctor to develop a plan to manage all the medications you take. By working together with you, your doctor and our pharmacist we can identify any issues that may arise with your medications.

PODIATRY

Podiatry services are provided onsite by Mia from Fully Feet Podiatry. This includes nail care, callus & corns, orthotic therapy, along with diabetic feet assessment. Hi-Caps for instant rebate claiming, plus Bulk Billing through Enhanced Primary Care (EPC). Ask your GP if you are eligible for BULK BILLED treatments.
